This is a correct variation of the 1969 voucher issue pattern - with zero numbering, diagonal printing and perforation MODEL, additionally perforated once with a round hole.
A very nice piece, with typical features such as paperclip holes (separately, sets of cent and dollar denomination patterns were stapled into booklets) and mild traces of rust at this point.
Voucher confirmed with a solid PMG grading note.
