This is a correct variation of the 1979 voucher issue pattern - with zero numbering, small stamp and perforation MODEL.
A very nice piece, with a typical feature of this type - paperclip holes (separately, sets of cent and dollar denomination patterns were stapled into booklets).
Voucher confirmed with a solid PMG grading note, this is the 2nd Max note in the PMG register.
